Eligibility
To be eligible for an eyeglass voucher, applicants must:
* Have household income at or below 250% of the U.S. Poverty guidelines.
* Have had a recent eye exam within the last 24 months.
* Have no other resources available to them to pay for glasses, including federal or state programs.
Application Instructions
Applications for glasses are recommended to be submitted by a social worker, case manager or other advocate but can be submitted by the individual themselves.
