Description

Provides long-term support programs that help people with disabilities as they live in the community.


Disabilities can include:
* Developmental disabilities
* Chronic medical conditions
* Acquired or traumatic brain injuries
* Physical disabilities


Provides such support services as:
* Assistive technology training and education
* Economic assistance programs - help with housing, nutrition and income
* Environmental modifications and provisions
* Personal support services - home care services, home delivered meals, personal care assistance
* Social Security Advocacy - help to apply or maintain benefits
* Health care programs - such as community dental clinics, chemical dependency services
* Trainings - providing life skills and employment opportunities
* And more

DB101 is a tool of the Disability Hub MN™ System. Disability Hub MN is managed by the Department of Human Services Disability Services Division, and the service is delivered through a contract with the Metropolitan Center for Independent Living (MCIL) and the Southeastern Minnesota Center for Independent Living (SEMCIL)
