Description

Services include training and assistance in independent living skills for adults with developmental disabilities or related conditions.

The goal of Semi-Independent Living Services (SILS) is to support people in ways that will enable them to achieve personally desired outcomes and lead self-directed lives in the community.

Services provided may include training and assistance in:
* First aid and obtaining assistance in an emergency
* Learning and exercising rights and responsibilities of community living
* Meal planning and preparation
* Obtaining and maintaining a home
* Personal appearance and hygiene
* Self-administration of medication
* Shopping
* Social, recreation and transportation skills, including appropriate social behavior
* Using the phone and other utilities
* Managing money
* Engaging in activities that make it possible for adults with developmental disabilities or related conditions to live in the community
* Provide supported employment in community work environment settings.

A SILS recipient may receive a one-time housing allowance of up to $1,500 to cover costs related to damage/security deposit for housing rental, utility deposit, household furnishings and other items necessary that will enable the person to secure a residence in which to receive SILS.

Eligibility

To be eligible to receive SILS, the adult (18 or older) must:
* Be determined to have a developmental disability or related condition(s) as defined by the state of Minnesota
* Be unable to function independently without SILS
* Not be at risk of placement in an Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD)
* Not be residing in an institution, ICF/DD, or nursing facility
* Not be receiving home and community based waiver services

Application Instructions

The county or tribal human services office will provide an assessment and determine eligibility for the program.

Fees

Costs for SILS services are funded by counties, tribal agencies, and the Minnesota Department of Human Services Disability Services Division. There are no recipient fees associated with SILS.

Programs that assist people who have disabilities to learn the basic skills of daily living through individual and group counseling and instruction, experience and practice in coping with real or simulated life situational demands; or through the use of assistive devices, special equipment and specialized assistants. Programs that provide training in social interaction skills for young children, youth and/or adults with the objective of helping them overcome shyness or aggressiveness, engage in constructive play or other group activities, develop positive peer relationships and feel comfortable in both business and social situations. Sessions may focus on politeness, cooperation, negotiation, problem solving, taking turns, sharing, winning and losing, sportsmanship, body language, eye contact, using appropriate language, telephone manners, peer situations, consequences of actions and initiating, conducting and concluding conversations.
