Eligibility
Patients must:
* Have a social security number and receive treatment in the United States
* Have valid insurance coverage
* Be diagnosed with a covered disease and program must be accepting enrollments
* Medication must be Food and Drug Administration (FDA) approved to treat the covered diagnosis
* Meet income program guidelines
